The Ninja mask is in the Islands level, the cardboard mask is in the Magician's Palace, and the wooden helmet is in the Wedding Reception - I'm not sure which one you were referring to.

I'm still not sure which one you mean - do you have a picture of the LBP costume you're asking about? I looked up tf2 spy but the closest I could see would be the ninja mask.

Oh - ok that's the cardboard mask with stickers put on it. The mask is in the Magician's Palace level, then just put square stickers in the same pattern as in the photo. Should work for you.

I'm pretty sure you just get it in the multyplayer part.
As for the sticker (my personal signature :3), you can't make it by arranging stickers. I used Create Mode (and grids) to make the pattern, took a picture of it with a cardboard background, and took a picture of it. (Don't completely copy it, though, because it's my personal signature, as you can see.)
I aced every level in 4 days or something. Unlocking everything will take considerably longer, though, due to multiplayer areas and such. XD
