What game makes you're skin crawl? It gives you nightmares and is sometimes so scary that you don't want to play.
For me, the scariest games I've ever played are silent hill games. 3 being the scariest for me; I never got in to 2, and "pyramid-head" didn't scare me at all. The voice acting is TERRIBLE though, so that is good for a laugh in between gameplay.
The idea is genius: It's scares come from stressful noises, disturbing monsters, depressing landscapes, and most of all: the fear of the unknown. Because people have an uncanny attracting to wanting to know more about stuff, SH uses this to its advantage. What do the monsters look like? What's behind this door? What was that sound? All of these things usually lead you to danger. The game is just freaking scary as heck.
The game also starts you off with these club arm things as the first enemy... AHHHHHHHH!!!!!

-The first Silent Hill game - the demo was scary enough for me.
-First Resident Evil game - dogs, giant snake, giant spider, Hunters (after first seeing those (in that FMV sequence), I didn't want to leave the save room).
-Project Zero - only played demo.
-Dead Space - especially those crazy fast ones onboard the crashed ship.
All I can think of for now.

Like the rest of you it was Silent Hill and Project Zero that scared me along with System Shock 2. I used to get scared more by movies than games, though.
There was also this game called Clone that scared me but it looks really dated now. It reminds me of that old Windows maze screensaver.

I love me some horror in movies and games, but this has scared me the most out of everything I've seen. It doesn't use cheap tactics either of having enemies jump out of nowhere all the time (though it does happen on the odd occasion), this game truly messes with your mind.

Hysteria Project - played on PSP in a bed, at midnight, when all the lights are turned out, my parents are sleeping and with headphones... I tried that, just to see if it's really so scary.

EDIT: Oh and I did the same with Dante's Inferno.

You know what would be a game they should make?
I am legend. That would be freakin scary.
Just think of it. The infected just jumping out at you unexpectedly. And all you have is a gun. All by yourself.
But they haven't made it into a game. I wish they would though.

You start having a blast killing all the infected ones with your amazing skills.
You're enjoying the campaign so much that the game isn't even scaring you.
And then the dog scene comes up.

Uninstalled, broke the game disc, sent it back to the developers, and cried in a corner for the rest of the day.

The only game I have played that actually gave my a scare is Minecraft, but that was more like a surprise scare, not a terror scare.
The game that gave me a terror scare was Ocarina of Time, with the "Dead Hand" boss, and that giant spider-flying-pineapple thing.ONWARDS TO THE MAGICAL LAND THAT THE INTERNET IS
